Thank you for your interest in being a part of the Central Harlem Health Revival. The faith-based institutions in Central Harlem are a critical part of making the Health Revival a success. If your organization is interested in participating in the week-long Health Revival kick-off and/or in the year-long activities, please fill out the form below.
