Faith-Based Registration
 
Thank you for your interest in being a part of the Central Harlem Health Revival. The faith-based institutions in Central Harlem are a critical part of making the Health Revival a success, and Harlem a healthier community. We invite you to join us in our 2010 events, and we want to support you in your health events by attending the event, helping out and /or collaborating when possible. If your organization is interested in participating in the Health Revival events, or in the year-long activities, please fill out the form below. Also, please let us know if you will participate in the Health Walk and Health Festival. We would love to have a number of your members participate in the health walk. This represents an opportunity to make a visual statement that our faith based institutions are concerned about body, mind, and spirit. Please join us for the walk and if you have one, bring a banner to display during the walk.
